I’m Dr. Ashmeet Bhatia, a geriatrician focused on medically complex older adults. Our practice operates outside the constraints of traditional insurance-driven medicine. By limiting our patient panel and structuring care around a membership model, we are free to spend the time your loved one’s care actually requires — in their home, on their schedule, without compromise.
This is not home health. This is your loved one’s primary care physician — fully equipped, fully present, and fully focused on them.

We begin with a 15-minute conversation — no commitment required. We learn about your loved one’s situation and answer every question your family has about our model.
Step 2
Comprehensive First Visit
An in-depth home visit with your loved one’s dedicated physician. Medical history, current medications, home environment, cognitive screening, and most importantly — what matters most to them.
Step 3
Personalized Care Plan
A tailored plan built around your loved one’s goals, values, and health needs — shared clearly with the patient and family, and revisited as circumstances evolve.
Step 4
Ongoing Partnership
Routine visits, direct physician access, proactive monitoring, and seamless coordination — year after year, through every stage of aging.
